Eric,
Is is possible to set up a default for the dimensions with the angle at 0? And with the white background?
I can change them after the fact, but it would be nice if I didn't have to!

I don't understand the first question.
For the second, yes you can set a white background to the texts of the dimensions.
Preferences of Dimensions -> "Line and text attributes" -> switch "Back" on and white color selected.

Currently, the angle of the dimension text is the same as the line: a horizontal is at A=0, vertical is A=90 or 270, diagonal is A=315 or 45. I would like the dimension text to always be A=0, regardless of the line angle or direction.

That is possible, switch off "Turn texts" in the Dimensions prefs.
